What documents will I need to apply for housing assistance?

(1) Application form

(2) Resident card issued from city, town or village of residence
a) All family members must be included
b) If you are from a foreign country you must also have your alien registration card (Gaikokujin touroku zumi shomeisho)

(3) Certificate for the last years income and tax amount deducted, issued by your city, town or village. Must include all family members with an income.
a) Between the months of January and May you may not be able to obtain these certificates. In this case you will receive a withholding slip or a final income tax return with an official stamp.
b) If you will be employed or you are wanting to change employment you will be asked to show a certificate of your salary (at least 1 month's salary)
c) Retirees and pensioners will be asked to show documents of pension or a pension notice

(4) Certificate of paid taxes issued by your city, town, or village.

(5) Copy of social insurance card for all family members.

(6) If you have no income you must show a certificate of proof
a) Certificate of taxation issued from your city, town or village
b) Employment insurance card issued by the Public Employment Security Office
c) Certificate of proof of the loss of health insurance and pension issued by the Social Insurance Office

(7) Other documents (not required for all cases)
a) Proof of engagement form
b) Proof of retirement plan from place of employment
c) Certificate of proof from the Department of Welfare
d) Certificate of proof of Disability
e) Certificate of proof of employment
f) If you are single and want to apply you must obtain a special application form
g) Other documents, etc.

(8) If you apply with (a) or (b) we will ask you for other documents after the application. In case of (a): You must show documents of proof after your marriage In case of (b): You must show proof of retirement

(9) When you apply for any time housing you must include a 140-yen stamp. With periodical housing you must include an 80-yen stamp.

*** With the certificate of taxation, you must have an official stamp dated within 30 days of application. The other documents must have an official stamp dated within 90 days of application.
